Case Manager – Registered Nurse (RN)

Position Overview

We are seeking an experienced Registered Nurse (RN) to join our team as a Case Manager. This role involves managing patient care efficiently and ensuring quality outcomes in an acute care environment.

Key Responsibilities

• Coordinate and manage patient care plans from admission to discharge.
• Conduct assessments and develop individualized care plans by collaborating with healthcare teams.
• Monitor patient progress and adjust care plans as needed.
• Utilize knowledge of Meditech, Midas, and Interqual in managing cases effectively.
• Serve as a resource for patients and their families regarding healthcare options and resources.
• Ensure compliance with regulatory standards and quality benchmarks.

Qualifications

• Must possess a valid Registered Nurse (RN) license.
• Minimum of 2 years’ experience in acute (hospital) case management required.
• Demonstrated knowledge and proficiency in Meditech, Midas, and Interqual systems.
• Strong communication and interpersonal skills, with a focus on collaboration.

Work Schedule

This position requires availability for every other weekend.
