Shift: 8 hour days, 08:00:00-16:00:00, every 3rd weekend

Job Description:
The Physical Therapist (PT) will provide therapeutic care to patients with a variety of conditions, including ortho, strokes, joints, and spinal injuries. The position will be primarily home-based care with assignments to a specific geographical region.
Responsibilities:
- Provide physical therapy treatments and evaluations.
- Work collaboratively with other therapy service providers.
- Maintain accurate and up-to-date patient records using the Cerner EMR system.
Requirements:
- BLS Certification required.
- Experience with Oasis form is mandatory.
- One year of experience in PT/OT/Speech.
- Home setting experience preferred but not required.
