Job Description:
Provide physical therapy services to patients within a 60-mile radius around Valley City, ND, primarily in a home health setting. Care for patients with conditions such as surgeries and falls. Operate within the HomeCare/HomeBase EMR system.
Responsibilities:
- Treat 5-7 patients per day in a home health and hospice setting.
- Travel within a 60-mile radius of Valley City, ND to provide physical therapy services.
- Maintain accurate patient records using the HomeCare/HomeBase EMR system.
Requirements:
- Active ND Physical Therapy License.
- BLS Certification.
- Current Driver’s License and Auto Insurance.
- Minimum of 1 year of PT-related experience, with previous home health experience required.
