Job Description:

We are seeking an experienced Outpatient Physical Therapist to provide rehabilitation services in a clinical setting. The ideal candidate will manage a varied caseload, employing skills in case management, screening, and assessment methods while utilizing the NextGen EHR system.

Responsibilities:

• Case management and building a caseload of 25-30 cases per week.
• Conducting fall risk assessments and gross motor assessments.
• Providing orthopedic care for a variety of conditions.
• Maintaining productivity tools and meeting a target of 10-15 visits per day.

Requirements:

• Must have 1+ years of experience in a similar role.
• Familiarity with EHR systems, specifically NextGen.
