Job Description:
The Physical Therapist will be responsible for providing home health care services, focusing on rehabilitation therapy. The role includes performing SOC visits, especially with orthopedic patients when nursing is not ordered.
Responsibilities:
- Delivering therapeutic patient care through home health services.
- Participating in SOC visits and managing a caseload of approximately 5 visits per day.
- Employing the EMR system HCHB – HomeCare HomeBase for charting patient data.
Requirements:
- BLS (AHA) certification.
- Experience in home health care strongly preferred.
