Job Description:
The Physical Therapist will provide rehabilitative care to patients in a home-based setting. They will work primarily Monday through Friday, with flexible shifts, and occasional weekend requirements. The role involves covering a designated geographical region and evaluating patients with conditions such as ortho, strokes, joints, and spinal injuries.
Responsibilities:
- Evaluate and treat patients with orthopedic, neurological, and other conditions requiring physical therapy.
- Document patient care using Cerner EMR system.
- Work collaboratively with a healthcare team, including rehab aides, OTAs, and PTAs.
- Adhere to policies regarding cancellations, charting, and attire.
Requirements:
- BLS Certification
- Experience with Oasis documentation
- Minimum of one year of experience in PT/OT/Speech, home setting experience preferred
