Job Description:
Physical Therapist responsible for providing rehabilitation services in a home-based care setting. The role involves working M-F with flexible shifts, primarily between 8AM and 5PM, with possible requirements for weekends and minor holidays if needed. Experience with Cerner EMR and Oasis is essential, along with BLS certification. Must cover assigned geographical regions and participate in a 1.5-2 week orientation.
Responsibilities:
- Assess and develop treatment plans for patients with orthopedic, stroke, joint, and spinal injuries.
- Collaborate with a multidisciplinary team including PT/OT/Aides to provide patient care.
- Ensure timely and accurate documentation using the Cerner system.
Requirements:
- Valid Physical Therapist license.
- BLS Certification.
- Minimum of one year experience in PT/OT in a home setting, Oasis experience preferred.
