Job Description:
Provide physical therapy services to home health and hospice patients within a 60-mile radius around Valley City, ND. Responsible for treating 5-7 patients per day with conditions such as surgeries and falls.
Responsibilities:
- Treat and manage home health and hospice patients.
- Use the HomeCare/HomeBase EMR system for documentation.
- Complete unit-specific orientation for 3-5 shifts.
Requirements:
- BLS certification.
- North Dakota Physical Therapy License.
- Current Driver’s License & Auto Insurance.
- Minimum 1 year of physical therapy experience and previous home health experience.
