Job Description:

Seeking a Physical Therapist for a home health department. This role involves managing patient care plans, performing start of care and recertifications, and conducting regular patient visits. Candidates must have a current driver’s license and auto insurance, and must follow a radius rule of 50 miles for employment eligibility.

Responsibilities:

• Perform start of care and recertification visits.
• Conduct 5-6 daily visits without additional travel or admission assignments.
• Provide safety evaluations or teaching visits in hospice situations if needed.
• Work collaboratively with support from the Clinical Coordinator and Director.

Requirements:

• Current BLS certification and ND Physical Therapy License.
• Preferred experience in Home Health and OASIS training.
• Experience managing a workload of 6.5 points per day.
• Black, navy blue, or grey scrubs are required.
