Job Description:
The Physical Therapist will be responsible for providing home-based physical therapy within a designated geographical region, focusing on patients with orthopedic, stroke, joint, and spinal injury diagnoses.
Responsibilities:
- Conduct physical therapy assessments and provide treatment in a home health setting.
- Coordinate with rehab aides and other therapy staff as necessary.
- Document patient progress using the Cerner EMR system.
- Adhere to the schedule of Monday to Friday and occasional weekends as required.
- Comply with business attire guidelines, including wearing a white lab coat.
Requirements:
- BLS Certification
- Experience with Oasis documentation
- Minimum of one year of experience in PT/OT/Sp
- Home setting experience preferred
- Ability to navigate a flexible schedule, including shifts on weekends and minor holidays as needed
