Job Title: PT (Physical Therapist)

City/State: Santa Maria, California

Shift: 8 hour days, flexible, majority of work hours are M-F 8am-5pm

Contract Length: 13 weeks

Job Description:

The Physical Therapist will provide home-based care to patients within a designated geographical region, attending to a variety of cases including orthopedic, stroke, joints, spinal injuries, and more. The role includes providing services aligned with rehab team practices in both speech and occupational therapy as needed.

Responsibilities:

• Provide physical therapy services to patients in a home health setting.
• Collaborate with rehab aides, OTAs, and PTAs, primarily permanent staff.
• Complete charting and documentation using the Cerner EMR system.
• Adhere to flexible scheduling with every third-weekend duty as required.
• Participate in floor-specific orientation for 1.5-2 weeks.

Requirements:

• Current BLS Certification.
• Experience with Oasis documentation forms.
• At least one year of experience in PT/OT/Speech, with home setting experience preferred.
