Job Description:
We are seeking a Physical Therapist to provide care in home-based settings, covering a geographical region. Flexible shifts primarily from Monday to Friday.
Responsibilities:
- Provide therapy and care as a PT in home-based settings.
- Complete documentation and charting using Cerner EMR.
- Participate in both inpatient and outpatient services as required.
Requirements:
- BLS Certification
- Experience with Oasis (government mandated form)
- Minimum one year of experience in PT/OT/Speech
