Home Health Nurse

Job Overview

This position involves providing home health care services to patients across four counties: Noble, Lagrange, Steuben, and DeKalb. The area is primarily rural with mid-sized towns, and the role requires a home health professional experienced in delivering quality care in patients’ residences.

Responsibilities

• Conduct Oasis Assessments and Start of Care cases.
• Provide care for common cases, including elderly falls and fractures.
• Manage daily patient visits using a tablet to track time and patient interactions.
• Maintain compliance with care plans and documentation standards.

Work Schedule

The position typically operates on a Monday to Friday schedule from 8:00 AM to 5:00 PM.

Patient Load

The facility utilizes a point system equivalent to 4-6 patient visits daily, ensuring a manageable workload for optimal patient care.

Dress Code

The dress code is business casual or includes hunter green scrubs paired with a white or black top.

Qualifications

• Must have prior experience in home health care.
• Experience with Home Care Home Base is required.
• A valid nursing license must be held upon application submission.
