Job Description:

Provide home health physical therapy treatments in various settings including homes, assisted livings, senior apartments, and board and care homes. Responsible for patient evaluations and start of care visits with a daily patient load of 3 to 5, and complete required documentation and case management.

Responsibilities:

• Conduct home health physical therapy treatments and patient evaluations.
• Supervise up to two different PTA/COTAs.
• Manage caseload with appropriate time allocation for travel and documentation.
• Participate in twice-monthly meetings for mileage approval.

Requirements:

• California PT license and BLS certification required.
• 2 years of experience in a healthcare setting, preferably home health or community health.
• Valid driver’s license and proof of automobile insurance.
• EMR HomeCare Home Base experience preferred; Home Health experience including OASIS required.
• Good oral and written communication skills; organizational skills.
