Long-Term Care (LTC) Nurse

Job Description

We are seeking a dedicated Long-Term Care (LTC) Nurse to provide high-quality care to residents in a nursing home setting. The ideal candidate will have a passion for enhancing the quality of life for individuals in our care.

Responsibilities

• Conduct comprehensive assessments of residents’ health and medical needs.
• Administer medications and treatments as prescribed by healthcare providers.
• Monitor residents’ vital signs and report significant changes to the healthcare team.
• Develop and implement individualized care plans for residents.
• Provide support and education to residents and their families regarding health issues and care plans.
• Collaborate with interdisciplinary teams to ensure the best possible care.
• Maintain accurate and up-to-date documentation of care provided.
• Ensure compliance with all regulatory guidelines and facility policies.

Qualifications

• Current nursing licensure in the state of practice.
• Experience in long-term care or related nursing settings is preferred.
• Strong communication and interpersonal skills.
• Ability to work collaboratively within a multidisciplinary team.
• Compassionate and patient-focused approach to care.
