Job Description:
We are seeking a qualified Physical Therapist to work in a home health care setting, providing rehabilitative services to patients with orthopedic, stroke, joint, spinal injuries, and various other conditions.
Responsibilities:
- Provide physical therapy evaluations and treatments in accordance with physician orders.
- Complete documentation using Cerner EMR system.
- Collaborate with occupational therapy and speech therapy teams as needed.
- Participate in a geographical region assignment.
Requirements:
- BLS Certification
- Experience with Oasis documentation
- Minimum of one year experience in PT/OT/Sp
