Job Description:
The Physical Therapist will provide services in a Home Health setting, mainly focusing on orthopedic/joint replacements and geriatric patients with multiple co-morbidities such as CHF and COPD.
Responsibilities:
- Perform SOC visits, especially with orthopedic patients when nursing is not ordered.
- Conduct up to 5 patient visits per day.
- Utilize HCHB (HomeCare HomeBase) for EMR documentation.
Requirements:
- Valid Physical Therapist license.
- Home Health experience required.
- BLS certification (AHA) required.
