Job Description:
The Physical Therapist will provide home health services focusing on therapy for patients, including those with orthopedic needs and geriatric patients with multiple comorbidities such as CHF and COPD. The role involves utilizing the HCHB EMR system, and SOC visits may be required, particularly for orthopedic patients when nursing is not ordered.
Responsibilities:
- Conduct five patient visits per day.
- Perform Start of Care (SOC) visits for orthopedic patients.
- Utilize HCHB – HomeCare HomeBase for charting.
- Work with the Intake RN and Manager within the office setup.
Requirements:
- Must have Home Health Experience.
- BLS Certification required.
- Valid license to practice as a Physical Therapist.
