Job Description:
Responsible for performing Oasis Assessments and Start of Care cases in a home health setting within four counties, serving primarily rural areas with mid-sized towns.
Responsibilities:
- Conduct 4-6 patient visits daily using a tablet for time and visit tracking.
- Assess and develop care plans for patients, focusing on fall prevention and rehabilitation of fractures, particularly in elderly patients.
Requirements:
- Current Physical Therapist license with willingness to wait for licensure submission.
- Basic Life Support (BLS) certification from the AHA.
