Job Description:

Provide physical therapy services in the Home Health for Therapy setting, working closely with orthopedic and geriatric patients. Conduct Start of Care visits, especially when nursing is not ordered.

Responsibilities:

• Perform Start of Care visits and routine visits for assigned patients.
• Utilize EMR (HCHB – HomeCare HomeBase) for documentation.
• Provide care for patients with conditions such as orthopedic/joint replacements and chronic illnesses like CHF and COPD.
• Coordinate with intake RN and manager for patient management.

Requirements:

• Home Health Experience required.
• BLS Certification.
• License to practice as a Physical Therapist in the state.
