Job Description:

The Physical Therapist (PT) will be responsible for providing home health services predominantly in a home health setting, with occasional duties in hospice for safety evaluations or teaching visits. The PT is expected to perform start of care, recertifications, and resumption of care for patients, managing a workload of approximately 5-6 regular visits per day, or 3 additional visits if an admission is involved. The PT will not work weekends and will be required to wear black, navy blue, or grey scrubs.

Responsibilities:

• Perform start of care, recertifications, and resumption of care evaluations.
• Manage a daily workload of approximately 6.5 points per day.
• Conduct regular home health visits and possibly some hospice evaluations or teaching visits.
• Engage with the Clinical Coordinator and Director for support in the field.

Requirements:

• BLS certification.
• Active North Dakota Physical Therapy License.
• Possession of a current Driver’s License and Auto Insurance.
• Preferred experience in Home Health and OASIS training.
