Shift: 8 hour days, 08:00:00-16:00:00, every 3rd weekend

Job Description:
The Physical Therapist will be responsible for providing rehabilitation services in home-based care to patients with various diagnoses including ortho, strokes, joints, and spinal injuries. The therapist will be assigned to a geographical region and will work primarily from Monday to Friday.
Responsibilities:
- Deliver physical therapy treatment in accordance with established standards and practices.
- Accurately document patient information and progress using the Cerner EMR system.
- Collaborate with Rehab Aides/OTAs/PTAs and other healthcare providers.
Requirements:
- BLS Certification
- Experience with Oasis documentation
- One year experience in PT/OT/Sp, home setting preferred
