Job Description:

Home Health Occupational Therapist responsible for providing evaluation and re-visits in north-eastern Sacramento, Folsom, and Woodland – Yolo county service areas.

Responsibilities:

Perform an average of four visits per day, including evaluations and revisits.
Document patient care using the Cerner charting system.

Requirements:

Two years of experience in a healthcare setting required.
BLS Certification and NPI number required.
Current California OT license required.
Valid driver’s license and proof of automobile insurance required for home health.
