Job Description:
The Occupational Therapist (OT) will provide home-based care focusing on rehabilitation services. Responsibilities include evaluating and treating patients with orthopedic, stroke, joint, and spinal injury conditions. The therapist will be assigned to a designated geographical region and will work primarily during Monday to Friday business hours, with every third weekend required.
Responsibilities:
- Evaluate and treat patients with various rehabilitation needs including orthopedic, stroke, joint, and spinal injuries.
- Utilize and complete government-mandated forms such as Oasis.
- Collaborate with Rehab Aides/OTA/PTA, predominantly perm staff, as needed.
- Adjust treatment plans based on patient progress and feedback.
- Participate in a 1.5-2 week orientation to familiarize with floor-specific procedures.
Requirements:
- BLS Certification required.
- Experience with Oasis forms.
- Minimum one year of experience in PT/OT/Speech therapy; home setting experience preferred.
