Occupational Therapist (OT)

Job Description

We are seeking a dedicated Occupational Therapist (OT) to join our Home Health team. The successful candidate will be responsible for providing high-quality therapy services in a home health setting, ensuring that patients achieve their rehabilitation goals.

Key Responsibilities

• Conduct evaluations and develop customized treatment plans for patients.
• Provide therapy sessions in a home setting, typically averaging four visits per day.
• Collaborate with other healthcare professionals to ensure comprehensive care.
• Document patient progress and adjust treatment plans as necessary.
• Educate patients and their families on strategies and exercises to enhance their recovery.

Qualifications

• Minimum of 2 years of experience in a healthcare setting is required.
• Current Occupational Therapy license in California.
• BLS Certification is mandatory.
• National Provider Identifier (NPI) number is required.
• A valid driver’s license and proof of automobile insurance are needed for this position.
• COTAs with home health experience may be considered if no OT applicants are available.

Work Schedule

This position is Monday through Friday, with daily hours from 8:30 AM to 5:00 PM. The therapist will be the first to float as required.

Travel Requirements

The coverage area includes northeastern Sacramento into Folsom and Woodland, with services provided in the Yolo County area. Mileage will be reimbursed at the GSA rate with prior approval.
