Physical Therapist (PT)

Job Overview

An experienced Physical Therapist is needed to provide home health therapy services throughout Curry County, Oregon. The successful candidate will evaluate patients, develop individualized treatment plans, deliver therapy services in patients’ homes, and coordinate care with the home health team.

Responsibilities

• Perform patient evaluations and develop tailored treatment plans.
• Deliver therapy services in patients’ residences.
• Collaborate with the home health team to ensure quality care.
• Manage a mobile patient schedule independently.

Requirements

• Active Oregon Physical Therapist license or ability to obtain before start date.
• Preferred experience in home health settings.
• Strong clinical assessment and documentation skills.
• Reliable personal vehicle and valid driver’s license.

Patient Volume

Clinicians can expect to make approximately 2-6 patient visits per day, varying based on visit type and patient location.

Travel & Mileage

• Clinicians will use their own vehicles for patient visits and will be reimbursed for mileage.
• Visits may be local to the area or require travel to northern Curry County.
• The longest one-way travel distance is approximately 69 miles for clinicians based in Brookings.
• Clinicians primarily assigned to North County are encouraged to secure housing in Gold Beach to minimize travel time.

Important Note

This position involves delivering home health services and requires travel to patients’ residences rather than working in an outpatient clinic.
