Physical Therapist (PT)

Job Overview

We are seeking a dedicated Physical Therapist (PT) to join our home health team in City, State. The role involves providing quality physical therapy services to patients in their homes.

Shift and Schedule

• Shift Type: Days
• Shift Length: 8 hours
• Guaranteed Hours: 40 hours per week
• Schedule: Monday to Friday, 8:00 AM to 5:00 PM

Patient Care

As a Physical Therapist, you will be responsible for managing an average of 4-5 patients per day during an 8-hour shift. You will address various diagnoses across all age groups, focusing on indications of multiple comorbidities.

Qualifications

• Minimum of 2 years of experience in physical therapy
• Current BLS certification
• Valid California PT license
• Valid driver’s license and car insurance
• Prior home health experience is strongly preferred

Job Responsibilities

• Provide regular physical therapy care to patients in their homes
• Complete clinical documentation within one day of patient visits
• Collaborate with other healthcare professionals as needed

Support and Resources

The management team is available 24/7 to provide guidance and support. Additionally, rehabilitation aides (COTAs) are available to assist with patient care.

Technology

You will utilize the HomeCare HomeBase (HCHB) for electronic medical record (EMR) practices.

Requirements for Onboarding

All onboarding modules must be completed prior to arrival. Failure to complete all modules will result in a delayed start date. No exceptions will be made.

Travel Considerations

Travel mileage is billable per the General Services Administration (GSA) rate.
