Physical Therapist

Position Overview

The Physical Therapist provides comprehensive physical therapy services to patients according to a physician’s plan of care. This role involves assessing patient needs, developing treatment plans, and collaborating with healthcare professionals to facilitate optimal patient outcomes.

Responsibilities

  • Assess and evaluate patient functional status, and develop individualized treatment plans in coordination with physicians.
  • Evaluate home environments to assess muscle strength, mobility, gait, and range of motion, and identify the potential for rehabilitation.
  • Instruct patients, families, and caregivers in the use and care of therapeutic appliances and establish priority needs for physical therapy.
  • Report changes in patient conditions and responses to treatment to the supervising physician.
  • Initiate physical therapy programs and instruct caregivers or other personnel involved in the patient’s rehabilitation.
  • Train patients in the use of prosthetic devices as appropriate.
  • Supervise home health aides involved in the patient’s rehabilitation program.
  • Identify additional home health service needs and refer patients as necessary.
  • Prepare and submit clinical and progress summaries, documenting patients’ conditions and treatment outcomes.
  • Provide physical therapy consultations to families and caregivers as needed.
  • Participate in peer consultations and ongoing staff competency assessments.
  • Comply with Title 22 Medicare Conditions of Participation, Medi-Cal, OSHA, and other relevant regulations.
  • Perform additional duties as assigned.

Qualifications

  • A minimum of two years’ experience as a Physical Therapist in an acute, home care, or rehabilitation setting.
  • Current certification in Basic Life Support (BLS).
  • Strong oral and written communication skills with good organizational abilities.

Service Area

This position services areas in Yolo County, including Woodland, Davis, Dunnigan, Arbuckle, West Sacramento, Natomas, Knights Landing, Capay, Vacaville, Winters, and Esparto.

Dress Code

Scrubs are not required; if not worn, professional attire such as slacks (khakis acceptable) and a polo or button-up shirt is expected. Scrubs, if chosen, may be of any color.

Additional Information

The role typically involves rotating through Saturday shifts once a month. Mileage reimbursement is offered at the GSA rate with prior approval.
