Physical Therapist (PT)

Job Description

We are seeking an experienced Physical Therapist (PT) to join our home health care team. The ideal candidate will provide high-quality physical therapy services to patients in their homes, addressing a variety of diagnoses and contributing positively to their rehabilitation and recovery.

Key Responsibilities

• Conduct assessments and develop individualized treatment plans based on patients’ needs.
• Provide physical therapy treatments to patients, ensuring adherence to their program.
• Document all patient interactions and treatment outcomes within one day of the visit.
• Collaborate with other healthcare professionals to optimize patient care.
• Assist patients of all ages, particularly those with multiple comorbidities.

Schedule

This position involves a regular schedule from Monday to Friday, 8:00 AM to 5:00 PM. Self-scheduling options may be available.

Qualifications

• Must hold a valid California Physical Therapy license.
• Current Basic Life Support (BLS) certification is required.
• Minimum of 2 years of physical therapy experience, particularly in home health settings.
• Valid driver’s license and car insurance are required.
• Experience working with patients of diverse age groups and conditions.

Additional Information

All required modules must be completed prior to arrival. Candidates will be pushed back if not completed, with no exceptions. Mileage reimbursement is available at the GSA rate.

Support

A manager/supervisor is available 24/7 to assist as needed.

Technology

The EMR system used is HomeCare HomeBase (HCHB).
