Home Health LPN/LVN

Job Summary

We are seeking a compassionate and dedicated Home Health LPN/LVN to provide quality care to patients in the comfort of their own homes. This role primarily involves routine patient visits and direct patient care.

Shift Information

• Shift Length: 8 hours
• Days: Monday to Friday
• Hours: 8 AM to 5 PM, with a one-hour lunch break
• No weekends or holidays required

Responsibilities

• Provide routine care to patients, focusing on wound care, wound vacs, and ostomy care.
• Administer IV antibiotics and manage PICC line care.
• Chart patient information using a Samsung tablet and maintain accurate records.
• Collaborate with the healthcare team to ensure high-quality patient care.
• No patient scheduling or case management required.

Required Qualifications

• Minimum of 2 years of experience in home health or equivalent field.
• Current BLS Certification (AHA).
• Valid driver’s license and insurance.
• Experience with HomeCare HomeBase EHR.
• Proficient in wound care, including wound vacs and ostomies.
• Experience with cardiac patients, including CHF and pneumonia.

Working Conditions

This position is within a home health setting, with no hospice care or floating to other facilities required. Scrubs are not mandatory; neat and appropriate casual clothing is acceptable.

Location

This position is based in a rural community located in northern California, offering great recreational opportunities, including mountains, lakes, and wilderness activities. It is approximately a four-hour drive north of San Francisco and about two hours north of Sacramento.

Orientation and Training

New hires will receive a dedicated orientation specific to home health practices, with a preceptor assigned to assist during the onboarding process. The orientation period is expected to last up to one week.
