Hospice/Medical Social Worker

Shift Information

Days (08:00 AM – 04:30 PM), no weekends

Department Overview

The Hospice teams provide essential support to patients and their family members during the final stages of illness. Services are delivered in a variety of settings, including patients’ homes, skilled nursing facilities, assisted living facilities, and adult foster homes. Offered services include:

• Intermittent skilled nursing visits
• Pain and symptom management
• Medical social services
• Counseling and spiritual support
• Hospice aides
• Trained hospice volunteers
• Bereavement support
• Medications and equipment as appropriate

Experience, Education, and Qualifications

• Oregon LMSW (Masters-level Social Worker) or CSWA (Clinical Social Work Associate) licensure required
• Healthcare Provider Level BLS certification required
• Valid driver’s license and auto liability/property damage insurance, with maintenance of vehicle in good working order
• Oregon LCSW (Licensed Clinical Social Worker) preferred
• Experience as a social worker in a hospital or medical setting is preferred
