Social Worker – Clinical Social Worker

Job Description

The Social Worker serves an essential role in supporting patients and their families as they navigate the emotional and practical challenges associated with illness and lifestyle changes. This position involves providing counseling and facilitating access to community, social, and hospital services. The Social Worker will also intervene as necessary in cases of suspected domestic violence or abuse.

Responsibilities

• Assist patients and families in adjusting to new health conditions and lifestyle changes.
• Conduct thorough discharge planning to ensure smooth transitions from inpatient care.
• Coordinate with healthcare teams to align patient care with individual needs.
• Provide counseling and support services for patients facing behavioral health challenges.
• Identify and report cases of suspected domestic violence or abuse professionally and sensitively.

Qualifications

• Master’s Degree in Social Work is required.
• Experience in medical social work, specifically with discharge planning, is preferred.
• Prior experience in inpatient behavioral health settings is strongly preferred.
• Excellent communication and interpersonal skills.
• Ability to work collaboratively with a multidisciplinary team.

Work Schedule

Shift: AM (08:00 – 16:30) – Full-time position requiring 40 hours per week.
