Licensed Clinical Social Worker (LCSW)

Position Overview

A dynamic opportunity for a Licensed Clinical Social Worker (LCSW) is available. The ideal candidate will possess a Master’s Degree and have at least 2 years of relevant experience, including travel experience preferred. This position involves a commitment to providing high-quality care and support to patients and their families.

Schedule

• Work Hours: 8 hours/day, Monday to Friday, from 8:00 AM to 4:30 PM
• Holiday Requirement: Available as needed
• Weekend Requirement: Not required

Key Responsibilities

• Conduct comprehensive psychosocial assessments and develop, implement, and adjust individualized treatment and discharge plans.
• Manage a full caseload of patients with moderate to high complexity, including those with co-occurring disorders and psychosocial instability.
• Provide crisis intervention and evidence-based therapeutic services across individual, group, and family settings.
• Lead discharge planning efforts, identifying and resolving barriers related to housing, insurance, legal status, and access to care.
• Coordinate complex transitions of care, ensuring continuity across inpatient, outpatient, and community-based services.
• Ensure compliance with regulatory, legal, and accreditation standards, including patient rights, mandated reporting, and documentation requirements.
• Manage court coordination and legal processes with accuracy and timeliness.
• Maintain high-quality documentation reflecting clinical decision-making and patient progress.

Qualifications

• Master’s Degree required.
• License and Certification: LCSW, Epic experience, BLS, CPI.
• Minimum of 2 years of experience required; travel experience preferred.
• New graduates will not be considered for this position.
