Outpatient Hospice/Medical Social Worker

Department Description

The Hospice teams provide essential support to patients and their family members during the final stages of illness. Services are rendered in various settings, including patients’ homes, skilled nursing facilities, assisted living facilities, and adult foster homes. Key services include skilled nursing visits, pain and symptom management, medical social services, counseling, spiritual support, hospice aides, trained hospice volunteers, and bereavement support. Necessary medications and equipment are provided as appropriate.

Experience/Education/Qualifications

• Oregon LMSW (Masters-level Social Worker) or CSWA (Clinical Social Work Associate) licensure is required.
• Healthcare Provider Level BLS certification is required.
• This position requires the use of the employee’s personal automobile. A valid driver’s license and auto liability/property damage insurance are required by law, along with proper maintenance of the vehicle.
• Oregon LCSW (Licensed Clinical Social Worker) is preferred.
• Experience as a social worker in a hospital or medical setting is preferred.

Shift Information

Days (0800-1630), no weekends.

Holiday Coverage

Holiday coverage is required.

Minimum Years of Experience

1 year of relevant experience is required.
